Initial Consultation

Your initial appointment will consist of a consultation and eye exam. Our optometrists will explain your diagnosis and treatment options and answer any questions you may have. Please plan to arrive 10 minutes early for your appointment and bring the following information with you:

• Insurance Information
• A list of medications you are presently taking including eye drops
• Current spectacles and/ or contact lens information

IMPORTANT: All patients under 18 must be accompanied by a parent or guardian.
